In a significant development for oncology research, a new randomized clinical trial has investigated whether higher doses of vitamin D3 can improve progression-free survival in patients with metastatic colorectal cancer.

The SOLARIS trial, published in the Journal of the American Medical Association (JAMA) and detailed in a report on August 6, 2026, sought to determine if escalating vitamin D supplementation alongside standard chemotherapy could yield better outcomes for late-stage patients.

The SOLARIS Clinical Trial Design

In this double-blind, multicenter, phase 3 clinical trial, 455 patients with metastatic colorectal cancer were randomly allocated to receive either high-dose or standard-dose vitamin D3 every 14 days alongside their standard chemotherapy regimen.

Patients in the high-dose group received an initial loading dose of 8,000 international units per day (IU/d), followed by 4,000 IU/d for subsequent 14-day cycles. Conversely, the 209 patients in the standard-dose group received 400 IU/d throughout the trial.

Overall Survival Results

The primary results of the study did not show statistically significant differences in progression-free survival between patients randomized to high-dose vitamin D compared to low-dose vitamin D.

Median progression-free survival in the high-dose group was 11.8 months, compared to 10.3 months in the standard-dose group. Furthermore, there were no statistically significant differences in the response rates of these patients’ cancers or in overall survival rates between the two cohorts.

A Potential Subgroup Benefit

Despite the overall neutral results, high-dose vitamin D3 did appear to benefit a specific subgroup of patients. Dr. Kimmie Ng, lead author of the study and professor of medicine at Harvard Medical School, noted that patients with left-sided primary colorectal tumors seemed to experience a benefit.

In this subgroup, those on the high-dose regimen had a median progression-free survival of 13.8 months, whereas for those on the standard dose, it was 10.2 months. Researchers caution that this finding is hypothesis-generating and requires further confirmation in dedicated trials.

As the medical community continues to explore adjunctive therapies for metastatic colorectal cancer, the SOLARIS trial provides critical data, emphasizing that while vitamin D is not a universal miracle cure, its targeted application may hold promise for specific tumor profiles.
